The meaning of baruch shem kevod malchuso l’olom va’ed is a prayer that the glory of HaShem’s sovereignty should be revealed (to the entire world) forever. If one did not think about the meaning, and has not yet started the possuk following it, one should repeat baruch shem with the proper kavana.
